ABSTRACT

SESSION TITLE: Critical Diffuse Lung Disease Cases 2 SESSION TYPE: Case Report Posters PRESENTED ON: 10/19/2022 12:45 pm - 01:45 pm INTRODUCTION: Acute eosinophilic pneumonia (AEP) is dramatic in presentation mimicking infectious pneumonia or acute respiratory distress syndrome in previously healthy individuals. Medications are a commonly recognized cause of AEP. Daptomycin, has been strongly linked to AEP. Herein, we present a case of a patient with a septic joint treated with Daptomycin who went on to develop AEP. CASE PRESENTATION: Patient is an 80 year old man with history of hypertension, hypothyroidism, atrial flutter, complete heart block status post pacemaker, who had a hx of a mucinous cyst on his left index finger, requiring hospitalization. Blood cultures were positive for MRSA s/p debridement of the joint. He was discharged on 4 weeks of intravenous daptomycin. Two weeks after being discharged he presented back to the hospital with fevers, fatigue and worsening shortness of breath. His temperature was 103.8 and O2 saturation of 90% on 2L NC. Laboratory findings included WBC count of 8.6 with no eosinophilia on differential, ESR 110, negative blood cultures, sputum cultures with commensal flora, negative urine legionella, PCR for SARS COV-2 was negative. Chest radiograph showed mild interstitial airspace disease in the left mid and lower thorax, along with small bilateral pleural effusions. CT chest showed scattered bilateral consolidations and ground glass opacities and trace bilateral effusions. Daptomycin was switched to Vancomycin. Patients oxygen requirements had increased to 6l NC. Patient underwent airway exam with bronchoscopy and broncheoalveolar lavage in superior segment of the lingula, which showed inflamed bronchial mucosa with copious secretions. Cell count of the BAL showed increased eosinophil count with negative gram stain and culture. Patient was started on methylprednisolone 60 mg four times per day and then tapered. Vancomycin was switched to oral linezolid. Patient's hypoxia improved and was discharged home on 3l NC. At four week follow up, he no longer required oxygen on ambulation and chest radiograph showed complete resolution of infiltrates. DISCUSSION: Over 140 drugs have been recognized as a cause of drug induced eosinophilic pneumonia (DIEP). The diagnosis of DIEP requires febrile illness <5 days, diffuse bilateral infiltrates, hypoxemia and BAL showing 25% eosinophils or eosinophilic pneumonitis on lung biopsy. Additionally, a diagnosis of DIEP requires exposure to a candidate drug in the appropriate time frame, exclusion of infectious causes of eosinophilic pulmonary opacities. It also requires clinical improvement after cessation of medication. Daptomycin has been strongly linked to DIEP. In 2010 US FDA issued a warning about the risk of developing eosinophilic pneumonia during treatment with Daptomycin. CONCLUSIONS: Daptomycin is strongly linked with DIEP. Clinicians should maintain a high index of suspicion for DIEP in patient treated with daptomycin who develop respiratory distress. Reference #1: Uppal, P., LaPlante, K.L., Gaitanis, M.M. et al. ABSTRACT

A prototype of a system for the detection of infectious human pneumonia pathogens based on multiplex solid-phase reverse transcription PCR (RT-PCR) was developed. Primers were designed to identify the DNA of six bacterial pneumonia pathogen strains, and the RNA of two viral pathogens of pneumonia: influenza A and SARS-CoV-2. The signal accumulation of elongated immobilized primers occurs due to the incorporation of fluorescently labeled nucleotides in the chain. The signal is detected after all the components of the mixture are removed, which significantly reduces the background signal and increases the sensitivity of the analysis. The use of a specialized detector makes it possible to read the signals of elongated primers directly through the transparent cover film of the reaction chamber. This solution is designed to prevent cross-contamination and is suitable for simultaneous testing of a large number of test samples. The proposed platform is able to detect the presence of several pathogens of pneumonia in a sample and has an open architecture that allows expansion of the range of pathogenic bacteria and viruses that can be detected.

ABSTRACT

BACKGROUND: Since December 2019, there have been cases of infectious pneumonia of unknown cause in Wuhan, Hubei Province, China. On January 12, 2020, the World Health Organization (WHO) named it COVID-19. There are few studies on the clinical characteristics of patients with COVID-19, and results vary widely in sample sizes. METHODS: Chinese and English databases were searched with "Infectious pneumonia", "COVID-19", "CT", "SARS-COV-2", and "Diagnose" as keywords. Rev Man 5.3 software provided by the Cochrane system was used to assess the quality of the included literature. RESULTS: Of the 18 included studies, ground-glass shadow was the most common computed tomography (CT) sign [95% confidence interval (CI): 0.79-0.97], followed by thickening of the blood vessels (95% CI: 0.63-0.78), and pleural thickening (95% CI: 0.02-0.15). Of the 18 studies, 12 reported that the lesions were externally subpleural (95% CI: 0.132-0.173), and 6 reported that the lesions were distributed in a single lobe (95% CI: 0.598-0.841). The heterogeneity test results showed that the morphology of the lesions was cord-like (95% CI: 0.092-0.172), grid-like (95% CI: 0.152-0.193), patchy (95% CI: 0.192-0.313), and nodular (95% CI: 0.591-0.745). DISCUSSION: Ground-glass opacity was a typical CT manifestation for patients with infectious pneumonia, and CT signs were instrumental in diagnosing this disease.

ABSTRACT

In order to improve the level of epidemic prevention and control, and strengthen the observation and monitoring of potential patients with viral pneumonia in isolated state, a medical intelligent monitoring collar based on DA14699 Bluetooth low-power chip was proposed. DA14699 chip is used as the main controller in the design scheme, and the temperature, cough and location information of potential patients are recorded and analyzed by its high-efficiency wireless multi-core processing ability. The LIS3DH three-axis acceleration sensor is used to judge the cough symptoms. The MLX90640 infrared sensor is used to continuously measure the body temperature. The L218 four frequency GSM / GPRS positioning module is used to complete the rapid and accurate positioning of personnel, so as to realize the comprehensive supervision of the implementation of home isolation measures. DA14699 chip supports Bluetooth BLE5.1 protocol. Epidemic prevention personnel can transmit and read the data recorded in the smart collar from a long distance, and display it directly on the intelligent Bluetooth handheld terminal, effectively avoid the risk of infection caused by close contact. Through the actual test, the monitoring function of the key parameters of the collar is reliable and has high application value.

ABSTRACT

Cases in the current novel coronavirus (COVID-19) pandemic continue to increase, bringing tremendous psychological pressures to frontline caregivers and threating the ability of existing medical care systems in many countries to cope with related demands. In this situation, nursing leaders have a heightened responsibility to assist nursing staff to remain at their jobs and feel secure, to strengthen safety systems, to provide adequate equipment and personnel training, and to proactively lead nursing staff. In this article, leadership strategies implemented under the COVID-19 pandemic are presented from the perspective of transformational leadership in nursing practice. This article is divided into the following five themes: 1. Leadership makes the vision more vivid; 2. Demonstrating charismatic leadership; 3. Leaders who stimulate intellectual potential; 4. Paying attention to spiritual inspiration; 5. Providing individualized sincere care. Leaders should promote the professional role of nursing staff and provide a safe and secure practice environment.
